ATTENTION:The Profile Submission Period is closed. If you have any questions about the status of your profile, contact your school editor (Go to Contact Us). If you have submitted your profile already, please check that your name appears on the Profiles Received page. If you submitted your information and your name is not there, contact your editor to resolve the problem.How to Submit Your Profile You have three choices for creating your profile for the yearbook: (1) submit the paper form that your received with your letter about the yearbook, returning it to the appropriate yearbook editor; (2) complete an online form; or (3) complete a Word form that you can download from this site, and email the completed Word form to form@nmh59.org. | Online FormYou can fill out an online form and submit it directly by clicking on the link below. When the form is completed, click on SUBMIT, and it will be sent to the Yearbook Committee for editing. You cannot save the form, so it must be completed in one session. This is best if you have a relatively brief profile to submit. You will get an email copy of your submission if you enter your email address.
